FY'03 50/50 Federal Reimbursement Program

Final Claim Verification Process

  • We have previously provided all districts with a listing of students for whom payments are currently being made under the 50/50 program. We have taken this information, verified it and processed a 60 percent reimbursement out of federal funds based upon this information.

  • We have now prepared a Final Claim Form file using the verified information mentioned above. This file contains three separate worksheets as follows:

    • Instructions
    • Payments
    • Cert_Sheet
  • This information can be accessed through the Department of Education's security portal (See separate download/upload instructions Download PDF Document Download MS WORD Document )

  • We ask that you complete all information for each student listed. Any changes to the information listed should be made below the student listing under "School District Corrections". The students SASID number must be referenced. For any changes to the verified student information, a signed 28M/4 form must be submitted along with the Certification Statement.

  • Once you have completed all information and/or made corrections, save the file as "03Res_finalXXX" (in place of the 3 question marks should be the districts LEA code) and please upload your file back to the secure portal no later than July 31, 2003. We cannot guarantee any payments for any files submitted after this date.

  • Based on the availability of funds, all districts that provide the requested verification on or before the due date of July 31, 2003 will be given a final grant payment by August 31, 2003. As with all grant payments, the payment will be sent to the municipal or district treasurer. The payment should be recorded in the municipal or district records as a receipt of grant funds, so that the funds will be available for expenditure by the school committee without the need for a local appropriation.

  • Questions concerning the completion of these forms can be directed to Elena DeMelin at (781) 338-6591 or edemelin@doe.mass.edu or to Bill Finn at (781) 338-6535 or wfinn@doe.mass.edu.

  • Questions concerning how to access the Security Portal should be address to Elena DeMelin at (781) 338-6591 or edemelin@doe.mass.edu.
