Instructions for FY'04 Special Education Reimbursement "Circuit Breaker" Program

Please download each file and save it on your hard drive.
Complete the Circuit Breaker Reimbursement Estimate Matrix Form (CB1) in accordance with the Circuit Breaker Reimbursement Estimate Matrix Instructions (CB2) .
Once you have completed all information and/or made corrections, name the file as 04CBXXX.xls (insert your district's 3-digit LEA code in place of the XXX).
Upload your file to the DOE secure portal no later than August 31, 2003. See SPED File Exchange Dropbox Instructions (CB8) .
Print a hard copy of the Special Education Circuit Breaker Certification Statement Form (CB3) , fill in the required information, sign it and mail to the address noted on the form.
It is very important that we receive the completed Matrix Form file and accompanying Certification Statement no later than August 31, 2003. Based on this initial filing, we will calculate the estimated state-wide reimbursement rate for FY04, and process the first payments to districts. These first payments will be made in October and will represent one-half of each district's estimated reimbursement for the year.
Requests for direct payments to private residential schools will be processed through the local school district. Districts must submit a Special Education Circuit Breaker Vendor Invoice Form (CB4) to certify that services have been provided to eligible students. We will not accept invoices for direct payment to residential schools until the services have been rendered and certified by the local school district. Payments to private special education residential schools will be made on a per diem basis in accordance with the days and rates as approved by the Operational Services Division. See Approved Special Education Private School Rate Listing (CB7) for the current per diem rates.
Please note that the direct payment option cannot be used for sole source placements, summer programs, and students with approved one to one services. For Circuit Breaker purposes, these students will be treated as day students, with all tuition payments made by the local district. Districts will be reimbursed for the state share.
We are presently scheduling Circuit Breaker implementation training through your local collaborative during the month of August. Once we have finalized the schedule, it will be published on our website.
