FY03 End of Year Pupil and Financial Report

Schedule S - Directions

Schedule S School Construction Project Data and FY03 Expenditures is an addendum to the FY03 End of Year Pupil and Financial Report. The Schedule should be filed in accordance with the time frame noted in the End of Year Directions. There are two parts to this schedule. Information requested in Part A should include all school construction projects currently on the School Building Assistance (SBA) Priority List. Part B should include all municipal or regional school district payments made on school construction projects from ban/bond proceeds. Attach additional pages as necessary. We hope to have this information available in a web-based form on the Department of Education's web site. When this web-based form is available, we will notify superintendents by e-mail through the Superintendent's list serve. Questions regarding this form should be directed to Doug Finnegan at 781-338-6540.

Specific Directions

Identify the name of the Local Educational Agency (LEA) and the LEA Code as assigned by the Department of Education. Regional School Districts and Union Superintendents should report information for all affiliated town projects.

  1. Priority List Projects (If your community does not have projects on the Priority List, proceed to Part B)

    • Project Name: Identify the name of the school construction project currently on the SBA Priority List. If the project is for a new school or the name of the school has changed provide both the name at application and the name by which the school is currently known.

    • Priority Rank Number: Projects currently on the Priority List should use the Priority Number on the FY03 SBA Priority List at: http://finance1.doe.mass.edu/sbuilding/priority03.html.

    • Type of Project: Indicate whether the project was approved as a new school, (New) addition and renovation, (A&R) renovation (Reno) or major repair of windows, roof, HVAC, accessibility or for safety issues (MR)

    • Actual Square Footage: Gross Square footage of school building

    • Dates:

      • Indicate the dates as requested. If project has actually started or been completed do not complete the "planned construction or completion" dates.

      • Post-Bid Form F filing is a requirement under School Construction Regulation Section 38.11 (2). If this information has been filed with the SBA Office, indicate the date of filing. If the information has not been filed, attach a completed, signed copy with this schedule.

      • Contractor Evaluation filing is a requirement under School Construction Regulation Section 38.12 (10)

    • Permanent Financing Interest Rate: Indicate the most recent Bond percentage for this project.

  2. Actual Total FY03 Construction Expenditures from Ban/Bond Sources: Indicate in this section, the total expenditures from ban/bond proceeds for ALL projects in FY03, regardless of whether the costs are reimbursed under the State's SBA Program. This information should reflect actual expenditures made to vendors in FY03, not bond payments. Do not include interest payments.
