Submission of Proposals and Related Information

Listed below is information that applicants need for proper submission of competitive grant proposals. Please share this information with all who will be involved in the submission process.

Standard Grant Application for FY2006

  • There is a separate signature page for each fund code.

  • The appropriate signature page is located within the individual Request for Proposals.

  • Applicant Agency Contact information needs to be submitted on the individual program Budget Detail Pages. Please provide all requested information, including the four-digit district code.

  • All FY2006 forms are included on the Grants Management web site http://finance1.doe.mass.edu/grants/.

Each proposal submitted should contain the following in the order listed:

  • Part I - General Descriptor Signature Page, including the amount requested for ;

  • Part II - Budget Detail Pages, including all applicant agency contact information;

    The Part II Budget Detail Pages should be stapled on top of the corresponding Part III Required Program Information pages.

  • Part III Required Program Information

  • SMARTT - Required elements

  • Statement of Assurances

(SMARTT)
Applicants must complete and submit the required elements of the SMARTT system with the Standard Application for Program Grants. Further information and directions can be found at http://www.doe.mass.edu/acls/smartt/default.html.

Applicants must submit the online SMARTT Plan by Monday, January 10, 2005 by 3:00 p.m.

Applicants must submit hard copies of the related elements of the SMARTT plan with the Standard Application for Program Grants.

The Letter of Intent to Apply Form must be received at the Department by 5:00 p.m. on Friday, November 19, 2004. It should be sent by Certified, Registered, or Return Receipt Requested mail.
